When clients complete the New Client Form, they’re redirected to your Booking Page.
They can choose an available time for:
A temperament evaluation
Private lesson
Daycare orientation
You get automatic notifications, and all documents (form + waiver + vaccine uploads) are attached to their client profile.

💬 Tip: Include screening questions in the booking form (e.g., “Has your dog ever shown aggression?”) so you can review before confirming.

We’re thrilled you’re interested in joining our community of dedicated dog owners. To ensure every dog’s safety, comfort, and success, all new clients must complete the steps below before their first class or daycare visit.
This form helps us get to know you and your dog so we can match you with the right program and support your goals.
You’ll provide information such as:
Contact and emergency info
Your dog’s age, breed, and personality
Health and vaccine history
Behavior background and training goals

For the health and safety of all dogs in our care, we require the following vaccines prior to participation in any service:
Rabies (per state law)
Distemper/Parvo (DHPP or DAPP)
Bordetella (Kennel Cough) within the last 6–12 months
Canine Influenza (recommended but not required)
Puppies must have received at least two rounds of core vaccines and be actively on their vaccination schedule to participate in group classes.
